Parent Forms

Parents of enrolled children for the 2011-2012 school year,
Please print and complete all of the forms on this zip file.  If you have more than one child, you must complete all of the forms for each child.  When they are complete, please return them to Hills and Falls in the self addressed manila envelope that is in your blue folder. Do not forget to include a xerox copy of your photo id AND the health form/immunization record that you get from your pediatrician.  All forms are due on FRIDAY, MAY 20th.  Please do not send your forms in unless they are all included in the envelope. 

*If your child has an allergy you must fill out the Food Allergy Action Plan and include a photo of your child.  If your child does not have an allergy you do not need to fill it out.
